I understand that water cremation is an irreversible process and I hereby authorize this pet for cremation. These form selections are preliminary and will be finalized on the invoice with payment.

*Pet Ashes: I authorize the crematory to scatter my pet’s ashes if the ashes are not collected within 90 days of the crematory's intake of my pet.
